HIPAA Notice of Privacy Practices

Our duty to protect your health information

Harmony Orthodontics is required by the Health Insurance Portability and Accountability Act (HIPAA) to maintain the privacy of your protected health information (PHI), to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How we may use and disclose your information

We may use and disclose your PHI for treatment (for example, coordinating care with your general dentist or other specialists), payment (for example, submitting claims to your dental insurance) and health care operations (for example, quality reviews and staff training). We may also send appointment reminders and information about treatment options.

Disclosures that require your authorization

Other uses and disclosures, including most marketing uses and any sale of PHI, will be made only with your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already relied on it.

Disclosures permitted without authorization

As permitted or required by law, we may disclose PHI to family members or others involved in your care (unless you object), for public health activities, to report suspected abuse, for health oversight, in response to court orders, to law enforcement, to avoid a serious threat to health or safety, and for workers’ compensation.

Your rights

You have the right to inspect and obtain a copy of your records, request corrections, request restrictions on certain uses, request confidential communications, receive an accounting of certain disclosures, receive a paper copy of this notice, and be notified of a breach of your unsecured PHI. You may restrict disclosures to your health plan for services you pay for in full out of pocket.

Changes to this notice

We reserve the right to change this notice. Any revised notice will apply to all PHI we maintain and will be available at our offices and on this website.

Questions and complaints

To exercise your rights or ask a question, call our Privacy Officer at (503) 666-2196. If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.

Effective date

This notice is effective as of May 1, 2026.
